Requirements
1. Application in EAP-I form. All particulars including name and
signature in the application should tally with those in the lost
passport
2. Damaged passport, in original, with a photocopy.
3. Five recent photos (Size 5cmx5cm) with white background and frontal
view.
4. Copy of the First Information Report (FIR) from the Police
authorities and its authentic legal English translation (This is
required where the passport is substantially damaged or it is damaged
in a fire/mishap. This document is not required if there is only a
minor damage)
5. A letter addressed to the Consulate, from the sponsor/employer
(from parent in case of a minor) of the applicant on the company’s
letterhead intimating about the loss of the applicant’s passport and
explaining circumstances leading to the loss of the passport. One
photo of the applicant should also be pasted on the letter. Following
format is suggested:
“This is‚to inform you that Mr./Os. ______________ s/o, d/o, w/o
_____________ holder of Indian passport No._____________ issued at
________________ on ______________ whose photograph is
pasted and stamped( company visa)/signed** below(, is
working under our sponsorship. He/She has genuinely lost his/her
above-mentioned passport. The passport was lost on ________________ in
the following circumstances _____ (Please explain in detail). Kindly
issue a new passport to him/her.**(by sponsor half on photo and half
on paper with sponsors passport copy)
6. Original with a copy of one or more of the following photo-affixed
identity cards to establish identity and nationality (The original
will be returned after verification):-
(a) Identity Card (b) Labour Card (c) Health Card (d) Driving License
(e) Election commission card
7. Newspaper advertisement, in original, in two leading local dailies
(one each in English and Arabic) as per the following format. Please
attach full sheets of the newspaper and not cuttings thereof (This is
required where the passport is substantially damaged or it is damaged
in a fire/mishap. This document is not required if there is only a
minor damage):
“Indian Passport No.___________ dated _________ issued at ____________
and held by Mr./Ms. ___________^_ s/o, d/o, w/o ____________________
has been genuinely damaged. The passport was damaged on ____________
in the following circumstances. Finder please contact
__________________,” 8. A Sworn Affidavit signed by the applicant
in the presence of a Consular Officer, explaining
a) The circumstances leading to the damage of the passport;
b) Details of travel on the lost passport;
c) Details of travel concessions including Transfer of Residence
benefits, Foreign Exchange availed of, imports made by the applicant
on the damaged passport;
d) Places of stay and work in UAE since arrival
9. Personal Particulars Form (in Triplicate with pasted
photos) in the case of passports )issued by other Passport
Issuing Authorities
10. Personal appearance of applicant is compulsory to establish
physical identity.
NOTE: IN THE CASE OF
APPLICANTS WHO ARE ON CASUAL VISIT TO UAE AND NOT A RESIDENT IN THIS
COUNTRY, HE/SHE WILL BE ISSUED AN EMERGENCY CERTIFICATE TO RETURN TO
INDIA.
